Apply at any AAA office or online.
How much? maybe $20.
Some countries will honor USA licenses for a short stay. Check them out. --99.59.x.xxx |

I used to have one of those. Just get a template online and print one out. There is no official issuing agency, and your State DL is required anyway, and accepted anywhere I've been. --47.216.xx.xxx |

Fred has it... Most countries do not require it in my experience, but it's a ticket when they do ;) --62.216.xxx.xxx |

What he means is: If you go to a country that requires an international license with your USA license only, they will ticket you.
I went to China, and was told to forget about driving in China, even with an international license. Some of the reasons:
- Signage is in Chinese.
- Insurance.
- A police officer is also on the spot judge if there's an accident, and he would rule against a foreigner (laowai) even if the visitor is not guilty. Money (or ransom) will get you off the hook. --99.59.x.xxx |
