Here is the text that is on mine. The formatting is going to be all messed up, but the two paragraphs at the end are in 2 separate columns. I think someone on here shared this a number of years ago. I may have tweaked it.

NOTICE TO RENEW LEASE OR VACATE

Date of notice: April 1, 2016

TENANT(S): John and Jane Doe

PREMISES:

123 spring St

Springfield, NY 11111

LEASE EXPIRATION DATE: May 31, 2016

RENT INCREASE: $35 increase

Instructions:

1. Please indicate if you wish to re-new your lease, or if you intend to vacate by the expiration of your current lease by checking either “Renew” or “Vacate”.

2. Be sure to sign and date this notice.

3. Return this completed notice to your landlord in the envelope provided by April 15, 2017 so that we may process your request.

Please indicate whether you wish to re-new or vacate

ˇ I Wish to Renew the Lease ˇ I will Vacate

Notice to Owner, Laura VA,

I agree to re-new our lease for a term of one year

at the rent of $1150 beginning June 1, 2017 and ending on May 31, 2018. I understand that the terms, rights and conditions in our original lease will still apply as agreed.

Another strategy advocated here is to give tenants some say in the rent increase to get buy-in and (possibly) a higher raise than you would have asked for.

Example. "It's that time of year for lease renewal. Due to rising costs we must adjust the rent so we can keep your home well-maintained. We value you as a resident and want to make this as bearable as possible. Please contact us no later than MM/DD/YYYY and let us know what you can afford."

If you were going to ask for $25 and they say $40 is their max, then you just got an extra $180/year for the cost of a stamp, a piece of paper, and printer ink. --173.19.xx.xxx

Seminars pay more than they cost! Get to Cleveland later this month.

I use Jeffrey's technique as SID mentioned. I recently sent out 47 increase letters and some "self increased" by as much as $150/mo!

Overall I gained a few thousand$$$ per month.
