In my second floor bathroom, there are mold/black spot showing up everywhere on celing. Mainly on top of the tub/shower area. I have an exhaust fan in the bathroom. What could be issue? Is the fan not doing it's job? --65.118.xxx.xxx |

It can still get humid enough in there even with an exhaust fan to grow mildew. This is especially true in the tub/shower area where the curtains/shower doors remain closed quite a bit, trapping humidity in.
Wipe the ceiling down with bleach or hydrogen peroxide or a mold remover.

Also hang up wet towels with the window cracked open slightly, it sounds like a slight shortage of ventilation, when the outdoor air humidity climbs up this symptom will appear and it is really a flash in the pan in comparison to having a overhead water leak from a shower curtain left open too far. --75.94.xxx.xxx |

Scrub it like Gail said. Wiping off mildew is everyday housekeeping and SHOULD be done by the resident, but they won't. Then they get tired of seeing the spots and move out, maybe sue you for helath problems due to mold.
So we clean it.
Check the exhaust fan. Many are clogged.
But I find exhaust fans do very little, especially in ceiling corners.
Some interior paints seem to almost BREED mildew.
With the resident's cooperation, set a small $10 fan in the the doorway and let it dry a few days. Then prime it with Zinsser BIN or KILZ, wait 24 hours, then paint it with EXTERIOR PAINT with mildew additive mixed in.
