State Specific Question About: VERMONT (VT)

I filed a complaint and summons in Wyndham Superior Court, but the Deputy Clerk said it appears my paperwork is set-up for Federal Court. I think it is merely the check was directed to U.S. District, but want to know is the "Civil Cover" sheet that I included required for eviction? Is there specific language in the complaint that is necessary? --72.70.xx.xxx
