If your jurisdiction is online, perhaps the form is available there.
Did you inquire at the court? What did they say? --75.17.xxx.xxx |

Jackson County has forms on line. You could use the appropriate one from there and change the county name, etc. to fit your situation.
Add a www before the link below

Go to your County Law library you should be able to research evictions there. Isn't it amazing how little service you get from the very offices that your tax dollars support
your other option is find a flat fee attorney and have them handle the eviction --70.244.xxx.xx |

That is ridiculous. "do your own research"... sounds like that is exactly what you were trying to do. You asked them for the form, not "whadda I do?" --66.228.xxx.xxx |

Check your yellow pages for a place that sells legal forms to lawyers. Not the Office Max/Depot etc. A regular legal publishing place. Probably located right on top of your county court house. --151.213.x.xx |

Call around for a FLAT FEE EVICTION ATTORNEY. ASAP. Do not DELAY.
Once, you have the attorney. DO NOT talk to the TENANT.
Sorry, and GOOD LUCK

the court should sell them - not selling them and not having them online would be incredible! --125.25.xxx.xxx |

Try to talk to a different person at the court house and ask them where you are supposed to find the forms. --76.231.xxx.xx |

Funny how the courthouse staff doesn't want to give "legal advice". I hear the judge give advice to deadbeats every time I'm at court. My favorite is telling tenants that it will be a couple of weeks before the sheriffs come out. --68.90.xxx.xxx |
