Read the SC State LL/Tenant laws (Blue link, upper left corner of this page). If you don't have the exact language of the 5 day notice in your lease, you must issue that first. If it's in your lease, go straight to the courthouse with the signed lease in your hand. Pay the filing fee ($35) and they will provide all the forms. I always pay for a process server to deliver the papers (Another $30, but he is fast and efficient, and it is added to what the tenant owes). If the tenant does not "cure" in the specified time, you have to go to the courthouse again and issue an ejection notice (separate document). You then schedule with the sheriff when you and your crew will be available to set all his stuff to the curb.
In SC you can accept partial payment and STILL set his stuff to the curb if not paid in full. Just make sure you don't collect the full amount you file for on the eviction notice.
