That is a pdf form. You need the right kind of Adobe software to modify it.
I don't have that software - and I don't use their form anyway because I always modify to request late fees.
Many years ago I just retyped the form to fit my needs. Then I update it as needed.
I haven't evicted someone for a while, so before I do I may need to update my form.
Be sure and check with the courthouse about how many copies you need. It depends on the number of defendants and if you also are going to request posting.
An advantage of posting is that you can get a judgment possession even if the tenant wasn't served personally. A disadvantage is that you can't get a judgment for rent unless the tenant is served.
In Jackson County, you get much better results on service if you have a special (private) process server. If you do, you need another form for that.
The reason you got such a quick response from me is I happened to still be up and at my computer, which I usually am not at this time. And whenever there is a question tagged as Missouri specific, it gets emailed to me. Each state has a poster who has agreed to do that.
